This a Number 8 3-way stub turnout, frog angle 7 degrees 9 minutes.
In surveyor, you will find it as Turnout Stub 3-way in the 'Buildings' menu.
Use the properties menu in surveyor to set the initial direction, the side the switch stand is on, and the texture of the various legs.
Track textures match my DB - series of track.
